What the official source says

Modelo 790, Código 012 is the Spanish administrative fee form for the foreigner procedures handled by the Policía Nacional. It is completed on the official Policía Nacional portal, which names it Tasa modelo 790 Código 012. Reconocimientos, Autorizaciones y Concursos, and downloaded already filled in so the fee for the procedure being applied for can be paid. It is an autoliquidación: the declarant selects the trámite and states the amount. The portal offers Principal and Complementaria, and beside Complementaria it prints "(Adjunte documento acreditativo del pago.)".

The portal groups the trámites this fee covers under: Tramitación de autorizaciones para la prórroga de la estancia en España; Tramitación de autorizaciones para residir España; Tarjetas de identidad de extranjeros (TIE) y certificados de registro de residentes comunitarios; Documentos de identidad y títulos y documentos de viaje a extranjeros indocumentados y otros documentos; and Tramitación de la solicitud de autorización o visado de tránsito en frontera y de visado de estancia en frontera. The trámite buttons within those groups include the TIE, the certificado de registro de residente comunitario, the asignación de NIE, and certificados or informes.

The page does not print the amounts in its text. It says: "Si deja unos instantes el puntero del ratón sobre el botón de cada trámite, se mostrará el importe unitario del mismo." So what a procedure currently costs is read from that trámite's own button, and that figure is the one entered in the Importe field. Fields marked with an asterisk are mandatory.

To produce the impreso, the portal asks for identification details (NIF/NIE or another listed identity document, name or company name, and address), the declarant's localidad and fecha, the Importe in euros, and a Forma de pago offering En efectivo, E.C. and Adeudo en cuenta, with a Código IBAN de la cuenta where that applies. It requires a CAPTCHA before the "Descargar impreso rellenado" control. Whether a procedure requires the proof of payment to be attached to its application is set by that procedure's own official page.
