What the official source says

The official Seguridad Social page identifies Modelo TA.1 for three marked requests: affiliation to Spanish Social Security, assignment of a Número de Seguridad Social, or variation of existing data. The page says the form must be completed in capital letters and lists the 07-2024 PDF with date 10/07/2024.

The request section asks the filer to mark the applicable request and identifies the Dirección Provincial or Administración of the Tesorería General de la Seguridad Social (TGSS) as the destination. The applicant section includes full name, sex, nationality, date and place of birth, identity-document type and number, any existing Social Security number, and address. The form presents DNI, Tarjeta de Extranjero, or passport choices; its instructions state that passport is for registrations of EU/EEA/Swiss citizens and refer to providing a NIE within three months of entry. The instruction cites Artículo 7 Real Decreto 240/2007, de 16 de febrero, sobre entrada, libre circulación y residencia en España de ciudadanos de los Estados miembros de la Unión Europea y de otros Estados parte en el Acuerdo sobre el Espacio Económico Europeo. For a data-variation request, the PDF says to state the cause and record the change in the relevant sections; the other sections are not completed except sections 1.1, 1.3, 1.4 and 1.5.

The PDF also includes a disability-degree field where applicable, a maiden-surname field in specified EU-national cases, and section 1.9 Datos telemáticos. The form presents a Sí/No choice beside the wording about accepting informative Seguridad Social communications. In an English translation of the official Spanish wording, the form says: "The entry of these details entails acceptance of informational communications from Seguridad Social." NEXO reports this translation of the official Spanish form; it does not request, collect, or determine consent for Seguridad Social communications, and it does not state what legal effect any selection has. The accompanying-document section names an identity document (DNI, Tarjeta de Extranjero, or passport) and, where applicable, a disability certificate. The form includes a preferred notification address and signature spaces for the applicant or legal representative and, when applicable, the employer.

The official receipt section states that the maximum period for issuing and notifying a decision is 45 days from entry in the competent TGSS register. It says the period may be suspended when deficiencies, documents, or other necessary elements must be requested, and in the other cases listed in art. 22.1 of Ley 39/2015.
