What the official source says

The EX-18 is the official form for applying for the Certificado de Registro de Ciudadano de la Unión Europea. According to the official Policía Nacional procedure page, the certificate records a citizen of the European Union, European Economic Area, or Switzerland in the Registro Central de Extranjeros when the person will reside in Spain for more than three months.

The Policía page states that the application is made personally within three months from entry into Spain, at the Oficina de Extranjería in the province where the applicant intends to stay or establish residence or, if unavailable, at the corresponding Comisaría de Policía. It states that the certificate is issued immediately and records the registered person's name, nationality, address, Número de Identidad de Extranjero (NIE), and registration date.

The official procedure lists the completed EX-18, a valid passport or national identity document, and evidence that the applicable registration conditions in article 7 of Real Decreto 240/2007 are met. If the identity document has expired, the page says to provide a copy and evidence that renewal has been requested. Both the official form and the Policía procedure identify Real Decreto 240/2007 as the regulation governing this registration procedure.

The procedure also lists proof of payment using Modelo 790, Código 012. NEXO links only to the official Policía generator; it does not state a fee amount, select a fee concept, complete the form, or make a payment.
